Primary Care Membership
Individual: $200/month
Couple/Two Adults: $350/month
Family (Two adults, and up to two children who are 26 years or younger): $500/month
Single Parent (One adult and one child who is 26 years or younger): $275/month
Add children who are 26 years or younger: $75/month
Discounts available for single young adults
All memberships are subject to a $ 150-per-person enrollment fee.
